To attend our clinic, you must answer NO the following questions;
Do you have a confirmed diagnosis of COVID-19?
Are you waiting for a COVID-19 test or the results?
Do you have new or worsening respiratory symptoms including one of the following; cough, sore throat, shortness of breath, nose, sneezing, post-nasal drip, loss of smell with or without fever?
Do you have any of the less typical symptoms, such as; fever only, diarrhoea, headache, muscle pain, nausea/vomiting
Are you a probable COVID-19 case?
Have you been at a current contact tracing “location of interest” at the times published?
Have you had close or casual contact with other people in the last 14 days who are probable or confirmed to have COVID-19?
You may respond to this questionnaire HERE no earlier than 24hours prior to an upcoming appointment. We will also send this questionnaire to you with your text reminder the day before your scheduled consult.
